Senior/Occupational Therapist

Reporting to the Day Care Manager, you will be responsible for providing excellent individualised rehabilitative care to patients in Day Care, Homecare and Inpatient to optimise their quality of life.

Responsibilities:
• Manage clinical caseload and support palliative rehabilitation practice (e.g. breathlessness management, prescription of equipment, collaborative goal setting) across the various care settings.
• Deliver evidence-based individualised care through holistic comprehensive assessment, treatment and evaluation of care.
• To work collaboratively with multi-disciplinary colleagues to meet patient needs.
• Participate in service review to drive service improvement.
• Contribute to the implementation and monitoring of adherence to relevant clinical guidelines.
• To be a resource and support to other team members and community professionals.
• Take initiative in pursuing life-long learning and personal development through reflective practice and engage actively with learning and development opportunities at Assisi Hospice.
• Supervise and assign appropriate treatment plans to therapy assistants.
• Maintain high quality patient documentation and records.
• Any other duties as and when assigned.

Requirements:
• Degree in Occupational Therapy from an accredited institution
• Must be fully registered with Allied Health Professions Council
• At least 4 years of experience in an acute hospital setting or community setting
• Bilingual and ability to communicate in local dialects preferred

• Team player with good interpersonal and effective communication skills
• Able to multi-task and work in a fast-paced environment
